$20 value when you remain enrolled for 3 months in the Be in Control program.
$50 value if you are asthmatic and remain enrolled for 6 months annually in Parkland Community Health Plan's free Be in Control program.
Reward Details
- All ages.
- Must have a diagnosis of asthma and be enrolled in the Parkland Be In Control program.
- $20 value after being enrolled in the Parkland Be In Control Program for 3 months.
- $50 value after being enrolled in the Parkland Be In Control Program for 6 months.
- 3 months of enrollment must be completed between September 1, 2023 and August 31, 2024 to earn reward points.
- 6 months of enrollment must be completed between September 1, 2023 and August 31, 2024 to earn reward points.
- Reward points are automatically awarded in about 3 months.
- Activity must be completed between September 1, 2023 and August 31, 2024 to earn this reward.
- Reward points can only be used to shop for items in the rewards catalog.
- Reward points cannot be used to purchase alcohol or tobacco. Reward points have no cash value and cannot be converted into cash.
