Earn Rewards by Visiting the Doctor

Sometimes, staying healthy takes a little extra effort – and it’s nice when you can get rewarded for it!
Parkland Community Health Plan’s Member Rewards program gives members the chance to earn reward points for completing health and wellness activities. Once you earn the points, you can use them to get gift cards and other items from a rewards catalog.
One way to earn rewards: visiting the doctor!
Earn a $20 reward value when you:
- Visit your primary care provider (PCP) within 90 days of joining Parkland Community Health Plan.*
- Get a Well-Adolescent checkup (ages 12–18).*
- Complete a follow-up visit within 8–30 days after being hospitalized for a behavioral health diagnosis.*
Earn a $25+ reward value when you:
- Complete more than five prenatal visits per pregnancy ($25 value).
- Visit your primary care provider (PCP) for a follow-up appointment within 7 days after being discharged from the hospital ($30 value).**
- Complete a follow-up visit within 7 days after being hospitalized for a behavioral health diagnosis ($30 value).*
Earn a $50+ reward value when you:
- Complete up to six Well-Baby checkups between birth and 15 months; complete up to two Well-Child checkups between 16 and 30 months (up to $160 value for completing all eight checkups).*
