Extra Dental Services

STAR Medicaid members (age 21 and older) can receive $300 per year for dental services such as cleanings, checkups, and X-rays.*

*Excludes CHIP and CHIP Perinate.

How It Works

• The $300 allowance is provided on a prepaid debit card.
• To request your card, visit PCHPRewards.com or call 1-855-651-5093 (TTY: 1-844-488-9731).
• Your card will be mailed to the home address we have on file within 12 business days after your request is submitted.
• The card is set up to work only at dental offices, based on the type of business (merchant category code).
• Most dental offices that accept credit or debit cards will likely be able to accept this card.
• Always check with your dental office when you make your appointment—or before receiving any services—to make sure they can accept the card.

Important Information

• If your plan coverage ends, any remaining funds on the card will no longer be available. (Unused funds may be reinstated if your coverage resumes, so keep your card.)
