Reward Details
- Reward available once per program year.
- Activity must be completed between September 1, 2025, and August 31, 2026.
- To earn reward points, you must either call us at 1-855-651-5093 (TTY: 7-1-1) to report DTaP / Rotavirus or report your vaccines online. You will not receive reward points for completing this activity unless you report it.
- Reward points can only be used to shop for items in the rewards catalog.
- Reward points cannot be used to purchase alcohol or tobacco.
- Reward points have no cash value and cannot be converted into cash.
