$20 value annually for completion of a diabetic (retinal or dilated) eye exam for members 15 years and older (excludes CHIP Perinate).
$20 value for completing the HB/A1c blood test once every 6 months for diabetic Members ages 18 and older (excludes CHIP Perinate).
Reward Details
- Ages 15 and up.
- Reward points are automatically awarded in about 3 months.
- Activity must be completed between September 1, 2024, and August 31, 2025 to earn this reward.
- Reward points can only be used to shop for items in the rewards catalog.
