Reward Details
- Checkup must be completed in your first trimester or within 42 days of enrollment in Parkland Community Health Plan.
- Reward available once per pregnancy.
- Activity must be completed between September 1, 2025, and August 31, 2026 to earn this reward.
To request a car seat, log in to the Member Portal and select "Value-Added Services" on the homepage.
