Diabetes Program Reward Details
- All ages.
- Must have a diagnosis of diabetes.
- Must be enrolled in the Diabetes Program for 6 months in a row, up to $50 annual reward limit.
- Must achieve A1C less than or equal to 8%.
Eye Exam Reward Details
- Ages 18 and older.
- $20 reward for completion of annual (retinal or dilated) diabetic eye exam.
Blood Test Reward Details
- Ages 18 and older.
- $20 reward for completing an HbA1c blood test once every 6 months, up to $40 annual reward limit.
General Reward Details
- Rewards are available in approximately 3 months.
- Activity must be completed between September 1, 2026, and August 31, 2027 to earn this reward.
- Rewards can only be used to shop for items in the rewards catalog (including gift cards, toys, household items, and more) and cannot be converted to cash.
- Rewards cannot be used to purchase alcohol or tobacco.
