Member ID Cards

You may not think about your health insurance ID card very often. After all, it probably spends most of its time in your wallet  until you, your doctor, or another medical provider really need it.

Your member ID card (or health plan ID card) is proof that you have health insurance. Health care providers use the information from your member ID card to confirm they are part of your plan’s network and to bill your health plan for your care.

Member ID Card - STAR

STAR members will get an ID card from us. Upon enrollment, each of your children in the STAR program will have their own card. If you have a baby, you may use your own Medicaid ID number for your newborn’s services until the baby’s enrollment is finalized and a separate ID card is issued. You will not get a new ID card every month. We will send you a new ID card if you change your primary care provider or if you move.

Member ID Card - CHIP

CHIP members will get ID card from us. You will not get a new ID card every month. We will send you a new ID card if you change your primary care provider, if you move, or if your copayments change.

Member ID Card - CHIP Perinate Newborn

CHIP Perinate members will get a ID card from us. The Member ID is the same as your child's CHIP ID number. If you have more than one baby, each baby will have a separate ID card.

How to Read Your ID Card

The front of the ID card shows important member information, such as name, member ID number, and date of birth. It also shows the name and phone number of your primary care physician.

The back of the card has more plan information and phone numbers to call if you need help.

STAR and CHIP Perinate services do not require copayments, so copayment information is not included on the card. CHIP ID cards show the copayments that apply to the plan.

How to Use Your ID Card

Always carry your ID card with you when you go to the doctor – or your child’s ID card, if the appointment is for them. Your doctor will ask to see it each time you get care.

What to Do if Your ID Card Gets Lost or Stolen
