Join Us for a Sports Physical at No-Cost
Do you have a student who lives in Dallas County and in
between 7th grade and 12th grade for the 2026-2027 school year?
Get ready for the upcoming sports season with a no-cost
physocal from Parkland Community Health Plan.
Who Can Attend?
Sports physicals are available for students living in Dallas
County who are entering 7th through 12th grade for the 2026-2027 school year. A parent or legal guardian must be present at the appointment. Register here: https://form.jotform.com/260966778091169
What to Wear?
Athletic gear
or loose-fitting clothes such as:
- Athletic
shorts, t-shirts and athletic shoes
- Females are
asked to wear sports bra or athletic tank
top underneath their t-shirt
- Glasses or
contacts should be worn if prescribed
- No caps,
jewelry, flip flops or sandals
